Off Campus Shipping of Radioactive Materials

Summary:  All shipments of radioactive materials from UC Irvine to other locations, domestic or foreign, must be reviewed, surveyed and certified by Radiation Safety staff from Environmental Health & Safety. 


Follow the steps below to ensure the timely processing of items to be shipped:

  • Allow sufficient processing time.
    • For domestic shipments,submit information at least one week prior to the anticipated shipping date.
    • For foreign shipments, provide at least two weeks of advance notice.
  • Print and complete the top section of the Shipping Radioactive Material Form
    (PDF file) or (Word File).
  • Supply the name of and a reliable means of contacting (phone, fax, e-mail address) the Radiation Safety Officer of the entity receiving the radioisotope shipment.
  • Arrange for a copy of the receiving institutions NRC or agreement state radioactive material license to be mailed or faxed to Radiation Safety.
  • Mail or fax the Shipping Radioactive Material Form and additional information to:
