UC Irvine Hot Works Permit Request Form

A Hot Work Permit is required for any temporary operation involving open flames or producing heat and/or sparks. This includes, but is not limited to: Brazing, Cutting, Grinding, Soldering, Thawing Pipe, Torch Applied Roofing, and Welding.
After picking up or printing the approved permit, ensure that the job's supervisor signs the permit, and post the permit on the jobsite for the duration of the job . After the job is completed, file the completed permit in department files.

I verify the above location has been examined and that the precautions checked on the Required Precautions Checklist have been taken to prevent fire, and permission is granted to perform this work.
